Care Coordination

We have Care Coordinators at all sites (St. Lawrence County & Franklin County). Each works with your doctor to meet your medical needs, no matter how complex they may be.

The services that case managers offer include, but are not limited to, the following:

• Payment assistance for medications.
• Assistance with setting up arrangements for transportation to and from appointments.
• Referrals to other local agencies, which are determined by the patient's need.
