We now offer COVID-19 Testing at our Canton Health Center, located at 4 Commerce Lane.

All testing is done via appointment only, please call the COVID Testing number to schedule a day and time to be tested.

IF YOU HAVE SERIOUS SYMPTOMS, SUCH AS A FEVER HIGHER THAN 100 DEGREES, A COUGH AND SHORTNESS OF BREATH, CALL YOUR MEDICAL PROVIDER FIRST, OR CALL 911.


HOW IT WORKS

Do I need to be a Health Center patient in order to be tested?

You do not need to be a patient of the health center to receive a COVID Test. If you have an order from your provider, or feel that you need to be tested, please fill out our Patient Registration Form and call (315) 379-8132 to schedule a collection appointment.

Do I need an appointment?

When is testing available?

Testing is available Monday through Friday from 8am-4pm.

What type of test is performed?

We currently offer both Offsite Testing (testing that is conducted at an offsite lab), as well as Rapid Testing, also known as Point of Care (POC) Testing, which is performed at our Health Center while you wait for the results. The type of test you receive will be determined by your healthcare provider.

What does the test cost?

For tests conducted at an offsite lab, the fee is $25.00.

For Point of Care (POC)/Rapid Testing, which is done at the health center while you wait, the fee is $60.00.

Again, the type of test you receive will be determined by your healthcare provider.

Do I need a referral from a medical professional?

No, referrals are not required.

What do I need to do prior to scheduling an appointment?

If you would like to be tested but are not an established patient of Community Health Center of the North Country, please complete the following form prior to scheduling a collection appointment.

If you are already an established patient of the health center, completion of the following form is not required. Call (315) 379-8132 to schedule your collection appointment.

If you would prefer to provide us the Registration Forms during your appointment, you may download each one at the links below and fill them out by hand, as opposed to submitting online, or you can obtain copies upon arrival at the health center.

Will my insurance cover the test?

Please check with your health plan to confirm before scheduling a test. If you have health insurance, you must bring your card with you to the test site.

If you do not have insurance, or if your insurance will not cover the test, you can still get tested. Nobody will be turned away for not having insurance. How is my sample for testing collected?

All tests are collected nasally (through the nose). Before your sample is collected, you will be asked to gently blow your nose into a tissue to remove any possible blockages. Our nurse will then insert a nasal swab into one of your nostrils and gently rotate the swab continously for approximately 10-15 seconds, before the swab is removed.

How and when will I get my results?

If your test was conducted at an offsite lab, you will be contacted by the collection nurse from the offsite lab or a member of our health center team. Currently, results are usually received within 5 days of the lab receiving the specimen.

For Point of Care (POC)/Rapid Testing, results are received within 30 minutes while you are at the health center.

If you are an established patient at one of our health centers, you may also have access to the results by logging into your Patient Portal. If you don't already have a Patient Portal account, please call the Health Center and a member of our team will assist you.

What do I need to bring with me?

If you are not a patient at one of our Health Centers, please bring your insurance card, proof of identification, and referral sheet if one was given to you by your provider's office. (Referral not necessary to receive test).

My child needs a COVID test to return to school. What do I do?

Please note that most schools require both a Provider Note as well as a Negative COVID test in order to return. Please schedule an appointment with your child's Primary Care Provider as well as scheduling a COVID collection.
