Community Health Center of the North Country offers integrated Behavioral Healthcare Services for both pediatric and adult Primary Care patients. Efforts are made for care to be targeted and brief. A healthy body requires a healthy mind, and a healthy mind requires a healthy body. Receive all the care you need in one convenient location. If transportation is a concern, all our locations are located on bus routes.

Behavioral Healthcare Services are covered by most medical insurance plans, including Medicare and Medicaid. Insurance is not required to be seen, and a sliding fee scale is available for those who qualify. We will not deny access to services based on inability to pay.

Please remember, you must be an established Primary Care patient to access Behavioral Health services at this time.

What types of services do you offer?

Our Behavioral Health team offers Psychotherapy, Substance Use Care Coordination, Medication Management, Screening and Treatment, and Assessments and Evaluations. Our team offers these services to children, adolescents, and adults.

Do you offer Telehealth?

Yes, we do! Telehealth not only allows you to receive the same quality care that we offer in our health centers from the comfort of your own home, but it also allows healthcare professionals to consult with each other on your healthcare concerns from different locations. Telehealth is not only convenient for patients and providers, it reduces visit costs, patient travel time, and increases access to care in our rural communities.

Do you offer same day appointments?

If you are experiencing a medical emergency, please call 911. If you or someone in your care is suffering a mental health crisis that you feel should be handled quickly, please call our health center immediately and our staff will do our best to make sure that you or the affected person will be seen by one of our providers as soon as possible. If the crisis occurs outside of normal business hours, please call the Reachout Hotline at (315) 265-2422.

Is now a safe time to receive in-person Behavioral Health services?

Yes, we follow all CDC and NYS Department of Health guidelines extremely closely, including disinfecting procedures and use of Personal Protective Equipment (PPE) in the areas of our health centers where needed.
