Virtually connect with your healthcare provider from the comfort of your own home!

 
Community Health Center of the North Country's new Telehealth Services allow you and your family to receive the same high quality healthcare that we provide within our health centers, but in the comfort of your own home or wherever you may be. Healthcare appointments can take place on your own personal computer, tablet or smartphone using audio and/or video. Services are available for both existing and new patients and can be scheduled by calling one of our four health centers.

We currently offer Telehealth visits for the following services:
 

  • Primary Care
  • Behavioral Health
  • Dental Care


Visits can cover anything from general healthcare/wellness, prescription recommendations and refills, mental health counseling, follow-up appointments, and certain urgent care conditions that can be assessed and treated over audio/video. Not all health concerns can be properly addressed via Telehealth services and may require attention in person. To properly assist you or your loved one, please call the health center and our staff can provide guidance on which type of appointment is appropriate for your needs.

Telehealth allows healthcare teams in different locations to consult with each other on your healthcare concerns. Telehealth is not only convenient for patients and providers, it reduces visit costs, patient travel time, and increases access to care in our rural communities.

HOW DOES IT WORK?

CHCNC uses a video conference service called Webex to conduct secure Telehealth appointments. Using Webex requires you to have access to either a computer, smartphone or tablet, internet access and an email address. If possible, connecting to Wifi during your visit will allow you to avoid mobile data charges.

If you do not have access to the equipment needed for a video consultation, please notify our staff when you call the health center to schedule your appointment.

Before your scheduled telehealth visit, you will receive instructions for accessing the visit via email. A staff member from the health center will call you a few minutes prior to the appointment to assist and coordinate setting up your telehealth appointment via Webex.

Sometimes Telehealth appointments cannot be completed because unexpected technical difficulties occur. In that case, the provider will call you back on the telephone, or you will be offered another Telehealth appointment.
