Diabetes Self Management Program

Community Health Center of the North Country's Diabetes Self Management Program is a 6 week program designed for people who are living with diabetes, are pre-diabetic, or anyone who is interested in learning more about diabetes.

Topics covered within the program include nutrition, physical activity, problem solving, short-term goal setting and medication usage, as well as learning to deal with the frustration and difficult emotions that come with living with a long term health condition. The program is designed to provide encouragement and support from both leaders and peers, to help with day-to-day management.

The Diabetes Self Management Program is FREE and open to all. You do not need to be a patient of the Health Center to attend the classes. Family members, friends and caregivers are all encouraged to attend.

Classes will be held in each community where our four Health Centers are located: Canton, Gouverneur, Ogdensburg and Malone. Light snacks will be provided.
