Administrative Assistant

Community Health Center of the North Country seeks candidates for a full time Administrative Assistant in our Jefferson County office based in Top of Square Mall on the lower level in Watertown. This position will be responsible for providing clerical support to the Consumer Directed Personal Assistance Program (“CDPAP”) in Jefferson County as well as back up support to the Health Center. Duties include but are not limited to preparing outgoing communications, answering and managing incoming calls and communications, meeting with new CDPAP employees and participants, and coordinating the day to day needs of the office. The position requires a High School Diploma or equivalent, clerical and office experience preferred, working knowledge of Microsoft Office programs, as well as excellent organizational, computer and information management skills.

Certified Coder

Our expanding network of Health Centers requires two full time Certified Coders. Job duties include, but are not limited to, reviewing and analyzing health records to identify relevant diagnosis, reviewing procedures for distinct patient encounters, and conducting a review of records to ensure compliance with coding and documentation guidelines as well as governmental regulations. This position requires a Certified Coder certification through AAPC or AHIMA and advanced computer skills to include: EMR, Word, Excel, and Access. Both positons will be based in Canton.

Case Manager

Case Manager

Our Canton Health Center is currently seeking candidates for the full-time position of Case Manager. This position will provide health education and outreach services to members of our community who are uninsured or underinsured as well as facilitate referrals for services for patients who present with needs or barriers to receive care. The Case Manager assists patients with our sliding fee scale program, navigating billing issues with area hospitals and provider offices, and helping patients overcome transportation barriers to attend appointments.
Requirements include: An Associate’s Degree in a health or human service field; knowledge of available community resources; excellent communication skills; and the ability to establish and maintain positive working relationships with patients, staff, and community organizations.

Corporate Compliance Officer/Director of Quality Assurance

Community Health Center of the North Country is currently accepting applications for a full-time Corporate Compliance Officer/Director of Quality Assurance. This position oversees the administration and implementation of the agency's Corporate Compliance Plan, conducts regular review of the Corporate Compliance Plan's effectiveness and propose modifications as deemed appropriate, ensures patient safety through the development and maintenance of systems to detect and manage patient adverse events as well as proactive measures to evaluate areas of risk in the health center, investigates alleged violations and cooperate with appropriate parties to handle violations promptly, conducts and coordinates QA reviews to assess compliance with statutory and regulatory requirements and assists in the development and maintenance of agency Policy and Procedures Manuals.

Position requirements include a Bachelor's Degree in Human Services or a health-related field, three (3) years experience in a health or human services field and knowledge of DOH, OMIG and OPWDD regulations.

Health Home Care Manager

Health Home Care Manager

We are seeking qualified candidates to join our team providing access to primary, pediatric, behavioral, dental, and optometry services across the North Country. This position conducts and schedules assessments, referrals, advocacy/supports, and education of patients and enrollees and care team members assuring the patient receives quality services to maintain optimum healthcare needs without barriers. Caseload will include children and adults.

Requirements: Bachelors of Arts or Science degree with two years of relevant experience; or a License as a Registered Nurse with two years of relevant experience; or a Masters Degree with one year of relevant experience; valid NYS driver’s license and reliable vehicle since travel is required; basic knowledge of Microsoft Office; ability to maintain good working relationships with others; strong verbal and written communications skills ; ability to make decisions independently when warranted; high level of professionalism and confidentiality when dealing with healthcare issues. On call availability a must .

Two years’ experience in human service/social work field, experience in Mental Health, Substance Abuse and medical information preferred.

Health Home Coordinator

Health Home Coordinator

Our Health Home program at Community Health Center of the North Country provides care coordination and care management, helping people with complex health needs receive the highly personalized, comprehensive, and coordinated care needed to stay healthy. We are currently seeking a Coordinator for this program who will be responsible for managing and coordinating program operations; providing orientation, training and supervision of program staff; assisting in facilitating program decisions; and acting as a liaison and resource to patients and staff.

We are seeking a Psychiatrist and/or a Psychiatric Nurse Practitioner to provide psychiatric evaluations, diagnosis with recommendations for treatment, prescribe medications as conditions indicate and conduct follow-ups with recommendations for any changes to Treatment Plans. Located in Canton, this position will be responsible for providing and documenting any participant-centered nutrition education responsive to the identified needs/interests of participants and promote healthy lifestyles plan, including physical activity options in the clinic and community. Requirements: Registered Nutrition and Dietetic Technician OR Registered Nurse with Bachelor of Science in Nursing, a Bachelor’s degree in an appropriate health-related field with at least six credit hours in nutrition.
