Assistant Care Manager

Assistant Care Manager

Community Health Center of the North Country seeks a motivated individual for the position of Assistant Care Manager for our Health Home Program. This position will perform outreach to enroll referred individuals to the program. He or she will conduct assessments, evaluate needs, establish a care plan, and maintain referrals to those enrolled. Duties also include supporting the activities of Care Managers by performing supportive clerical tasks to maximize the time Care Managers can be serving clients in the field. A Bachelor’s degree in psychology, sociology, human services, healthcare, or related field with 1-3 years of experience preferred, or related field with 3-5 years of experience. Volunteer or Paraprofessional experience will be considered. Certified Coder

Our expanding network of Health Centers requires two full time Certified Coders. Job duties include, but are not limited to, reviewing and analyzing health records to identify relevant diagnosis, reviewing procedures for distinct patient encounters, and conducting a review of records to ensure compliance with coding and documentation guidelines as well as governmental regulations. This position requires a Certified Coder certification through AAPC or AHIMA and advanced computer skills to include: EMR, Word, Excel, and Access. Both positons will be based in Canton.

Corporate Compliance Officer/Director of Quality Assurance

Corporate Compliance Officer/Director of Quality Assurance

Community Health Center of the North Country is currently accepting applications for a full-time Corporate Compliance Officer/Director of Quality Assurance. This position oversees the administration and implementation of the agency's Corporate Compliance Plan, conducts regular review of the Corporate Compliance Plan's effectiveness and propose modifications as deemed appropriate, ensures patient safety through the development and maintenance of systems to detect and manage patient adverse events as well as proactive measures to evaluate areas of risk in the health center, investigates alleged violations and cooperate with appropriate parties to handle violations promptly, conducts and coordinates QA reviews to assess compliance with statutory and regulatory requirements and assists in the development and maintenance of agency Policy and Procedures Manuals.

Position requirements include a Bachelor's Degree in Human Services or a health-related field, three (3) years experience in a health or human services field and knowledge of DOH, OMIG and OPWDD regulations.

Digital Media Content Coordinator

Digital Media Content Coordinator

Our team is accepting applications for a full time Digital Media Content Coordinator. Can you design amazing digital campaigns and track the results? Working closely with the Director of Communications and Marketing this person will help develop and coordinate digital marketing strategy for both the CP and health center sides of the organization. The job duties include but are not limited to: managing content development for all social media platforms and agency websites, driving online engagement with all agency brands, and tracking and analyzing user behavior to report effectiveness of campaigns. Requirements include: Associate Degree in public relations, marketing, communications, or related field (Bachelor’s degree preferred), minimum of 3 years’ content development experience for digital platforms, and knowledge of Adobe Creative Suite. Experience in a healthcare environment and with Google analytics a plus!

Outreach and Enrollment Coordinator

Outreach and Enrollment Coordinator

Community Health Center of the North Country seeks candidate for an Outreach and Enrollment Coordinator. This position is responsible for assisting patients with insurance enrollment applications, insurance eligibility and outreach activities. This person must be comfortable speaking to groups, organizing enrollment events and activities and have a good understanding of insurance programs’ eligibility requirements. Candidates must have an Associate’s Degree in a Health or Human Service field, a Bachelors Degree preferred; experience in working in a medical office and performing duties pertaining to eligibility or enrollment with Medicare, Medicaid and third party payers preferred. Must be proficient in MS Word, Excel and become proficient in electronic medical records. Psych/Psych NP

We are seeking a Psychiatrist and/or a Psychiatric Nurse Practitioner to provide psychiatric evaluations, diagnosis with recommendations for treatment, prescribe medications as conditions indicate and conduct follow-ups with recommendations for any changes to Treatment Plans. The successful candidate will be part of a comprehensive, multidisciplinary health care team made up of licensed clinicians, LCSWs, nurses and case managers and will work with the team to provide quality care to the individuals served.

Ward Clerk

Ward Clerk

Performs clerical duties for our Canton health center by interacting with providers, nurses, and health center personnel to ensure patient documentation, file maintenance, authorizations, referrals and other patient/health center needs are being met. High school diploma or GED plus one year of experience in a medical-related field or office setting, or specialized education in a medical or business/office field required.  One-year may be a combination of experience and education. Knowledge of EMR also preferred.

WIC-CPA

Community Health Center of the North Country seeks enthusiastic candidates for the position of a WIC CPA. Located in Canton, this position will be responsible for providing and documenting any participant-centered nutrition education responsive to the identified needs/interests of participants and promote healthy lifestyles plan, including physical activity options in the clinic and community. Requirements: Registered Nutrition and Dietetic Technician OR Registered Nurse with Bachelor of Science in Nursing, a Bachelor’s degree in an appropriate health-related field with at least six credit hours in nutrition. A valid driver’s license is required, in addition to flexibility in work hours, which include working evenings, Saturdays, and traveling to trainings as assigned. Must be available to travel between St. Lawrence and Franklin Counties.
