Administrative Assistant

We have just started the search to find a motivated individual for the position of Administrative Assistant in our Canton Health Center. This position will monitor, screen, respond to, and distribute incoming communication including calls and emails for the Chief Operating Officer, Clinical Development Manager and the Practice Manager. The Administrative Assistant coordinates travel, meeting, and event arrangements as necessary. An Associate’s Degree in an office or business field required with 1-3 years of office experience preferred.

Certified Coder

Our expanding network of Health Centers requires two full time Certified Coders. Job duties include, but are not limited to, reviewing and analyzing health records to identify relevant diagnosis, reviewing procedures for distinct patient encounters, and conducting a review of records to ensure compliance with coding and documentation guidelines as well as governmental regulations. This position requires a Certified Coder certification through AAPC or AHIMA and advanced computer skills to include: EMR, Word, Excel, and Access. Both positons will be based in Canton.

Corporate Compliance Officer/Director of Quality Assurance

Corporate Compliance Officer/Director of Quality Assurance

Community Health Center of the North Country is currently accepting applications for a full-time Corporate Compliance Officer/Director of Quality Assurance. This position oversees the administration and implementation of the agency's Corporate Compliance Plan, conducts regular review of the Corporate Compliance Plan's effectiveness and propose modifications as deemed appropriate, ensures patient safety through the development and maintenance of systems to detect and manage patient adverse events as well as proactive measures to evaluate areas of risk in the health center, investigates alleged violations and cooperate with appropriate parties to handle violations promptly, conducts and coordinates QA reviews to assess compliance with statutory and regulatory requirements and assists in the development and maintenance of agency Policy and Procedures Manuals.

Position requirements include a Bachelor's Degree in Human Services or a health-related field, three (3) years experience in a health or human services field and knowledge of DOH, OMIG and OPWDD regulations.

Health Home Program Manager

Health Home Program Manager

Our growing network of health centers means our team is now seeking a Health Home Program Manager to oversee our Health Home Program. This position manages case assignment and capacity as well as providing guidance to internal staff on NYS DOH Health Home Policy and Procedures for Health Homes Serving Adults, which includes special populations of HARP members and Health Home Plus.

A Bachelor's degree in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreational therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other related human services field; OR a NYS teacher’s certificate for which a bachelor’s degree is required; OR NYS licensure and registration as a Registered Nurse and a bachelor’s degree; OR a Bachelor’s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses; OR a Credentialed Alcoholism and Substance Abuse Counselor is required, plus two years of experience.

Optician

Optician

We are also looking for a per diem Ophthalmic Dispenser (Optician) in our Canton Health Center. The candidate selected for this position will work 3 days per week and will assist our optometrist with patients, schedule patient appointments, and dispense eyeglasses. A New York State license in Ophthalmic Dispensing is required. Interested applicants must have ability to maintain a high level of professionalism and confidentiality in dealing with patient information and/or issues. 1 year of experience is preferred.

Performance Improvement Analyst

Performance Improvement Analyst

We are seeking candidates for the position of Performance Improvement Analyst. This position is responsible for the day to day implementation of the Health Center’s Clinical Quality Management Program and associated Work plan. The Performance Improvement Manager assists in the implementation and measurement of Patient Centered Medical Home, Pay-for-Performance, Uniform Data Set, Patient Satisfaction and other clinical quality initiatives. Requirements: Associates or Bachelor’s degree in healthcare field, a minimum of 3 years’ experience in healthcare and/or quality initiatives.

Psych/Psych NP

We are seeking a Psychiatrist and/or a Psychiatric Nurse Practitioner to provide psychiatric evaluations, diagnosis with recommendations for treatment, prescribe medications as conditions indicate and conduct follow-ups with recommendations for any changes to Treatment Plans. The successful candidate will be part of a comprehensive, multidisciplinary health care team made up of licensed clinicians, LCSWs, nurses and case managers and will work with the team to provide quality care to the individuals served.

Qualified Nutritionist-WIC

Qualified Nutritionist-WIC

The Qualified Nutritionist for our WIC Program is a professional who supports the mission, goals and objectives of Community Health Center of the North Country by improving nutritional services in the WIC Program. This staff person will follow standards for the assessment process used to determine WIC eligibility, and personalize nutrition education, high risk care plans, referrals, and tailoring of food packages. Position based in Malone at Community Health Center of the North Country.

Requirements:
Bachelors, Masters, or PhD in Nutrition, along with flexibility in work hours which includes working evenings and Saturdays and attending trainings as assigned, and a valid driver’s license for travel between St. Lawrence and Franklin Counties.
