Care Coordination Nurse

Care Coordination Nurse

The Care Coordination Nurse will be part of a healthcare team supporting general care processes alongside a number of providers. This nurse will perform care coordination and professional nurse functions to assess the severity of a patient's expressed health concerns. They will also serve as a liaison between health home care managers, community health workers, hospital care transition staff and primary care providers to ensure patients with chronic care conditions receive quality services, meeting their healthcare needs. This nurse will be responsible for the delivery of care within their scope of practice according to all applicable regulations while employed at Community Health Center of the North Country. Community Health Center of the North Country is currently in search of a Document & Referral Specialist to join us in our Canton health center. This member of the team will manage all referrals, including but not limited to: sending referrals, obtaining patient appointments, and retrieving patients' notes after appointment date. Duties also include processing diagnostic imaging and procedure orders within the health center, completing prior authorizations for medical procedures, tests and durable medical equipment (DME), and comprehending coverage under patients' insurance plans, such as knowing which procedures and tests require insurance authorization. This position also requires receiving records from external facilities and notifying the appropriate medical staff (nurse, clinician) of results, and retrieving records from hospital/emergency department per provider's request. Requirements include a High School Diploma or GED, one-year experience in a medical-related field or office setting, or specialized education in a medical or business/office field (one-year may be a combination of experience and education). Medical terminology knowledge preferred.

Requirements: Registered Nutrition and Dietetic Technician OR Registered Nurse with Bachelor of Science in Nursing, a Bachelor's degree in an appropriate health-related field with at least six credit hours in nutrition.
