Our Gouverneur Health Center is currently seeking candidates for a full-time position of Case Manager. This position will provide health education and outreach services to members of our community who are uninsured or under-insured, as well as facilitate referrals for services for patients who present with needs or barriers to care. The Case Manager assists patients with our sliding fee scale program, navigating billing issues with area hospitals and provider offices, and helping patients overcome transportation barriers to attend appointments.
Requirements: An Associate's Degree in a health or human service field, knowledge of available community resources, excellent communication skills, and the ability to establish and maintain positive working relationships with patients, staff, and the community.

Community Health Center of the North Country seeks enthusiastic candidates for the position of a WIC CPA. Located in Canton, this individual will be working mainly with infants, pregnant, breastfeeding and postpartum mothers, focusing on nutrition and heights/weights. This person will be responsible for providing and documenting any participant-centered nutrition education responsive to the identified needs/interests of participants and promote healthy lifestyles plan, including physical activity options in the clinic and community. Requirements: Registered Nutrition and Dietetic Technician OR Registered Nurse with Bachelor of Science in Nursing, a Bachelor's degree in an appropriate health-related field with at least six credit hours in nutrition. A valid driver's license is required, in addition to flexibility in work hours, which include working evenings, Saturdays, and traveling to trainings as assigned. Must be available to travel between St. Lawrence and Franklin Counties.
Our team currently seeks candidates for a Qualified Nutritionist for our WIC program, based out of our Malone location. This individual will support the mission, goals and objectives of Community Health Center of the North Country, by improving nutritional services in the WIC Program. This staff person will follow standards for the assessment process used to determine WIC eligibility, and personalize nutrition education, high risk care plans, referrals, and tailoring of food packages.
Requirements: Bachelors, Masters, or PhD in Nutrition, along with flexibility in work hours which includes working evenings and Saturdays, attending trainings as assigned, and a valid driver's license for travel between St. Lawrence and Franklin Counties.
Registered Dietician (RD/RDN) preferred.
