Our Eligibility Requirements:

• Must live in St. Lawrence, Jefferson or Franklin County
• Be enrolled in Medicaid or Managed Medicaid (a health insurance plan that contracts for CDPAP services)
• Prescription for home care from your doctor

In essence, CDPAP is a self-empowering, non-traditional Medicaid Home Care Program. Independent individuals with a disability can hire, train, schedule and supervise care attendants of their choosing.
