Health Home Care Coordination

A Health Home is not a physical place, more so a group of healthcare and service providers working together to ensure people in need receive the care and services required to stay healthy. Once enrolled, an individual will have a dedicated care manager working side by side with them to develop a highly personalized, comprehensive, and coordinated care plan. This plan will map out services the person may need, such as:


• Referral to primary, pediatric, behavioral, eye, and dental care providers regardless of insurance status or ability to pay

• Substance abuse care coordination

• Help with prescription medications

• Insurance enrollment assistance

• Help with food, benefits, transportation, and housing

• Access to various Community Programs

• Women, Infants, and Children (WIC) benefits


To be eligible for Health Home Services an individual must be enrolled in Medicaid, and have two or more chronic conditions, one single qualifying chronic condition such as HIV/AIDS, Serious Emotional Disturbances (SED), or Complex Trauma.
